This medium is for non-emergent interchange between patient and physician. We cannot answer questions of an emergent nature by e-mail. In an emergency, call your physician or your local emergency assistance number.

This e-mail communication is intended for PPCP patients only. If you would like more information about becoming a PPCP patient please call the office closest to your location. See "Meet Our Staff" page.

Be advised that the information you send through the internet cannot be guaranteed as secure. Do not include material of a sensitive nature in your message, and do not send any personal information from a computer that is either shared with others or accessible in any way by someone else.

If it takes you more than 5 lines to describe your problem you probably need to call the office to discuss your issue with the staff.

This e-mail is appropriate to use for the following:

1. Prescription refills. (Include Patient Name, Insurance, Pharmacy, Pharmacy phone number, Drug, Drug dose, number of pills per script, and frequency.)
2. Appointment scheduling.
3. Need for referral. (Must have 5 days to process referrals.)
4. Insurance questions.
5. Follow-up information per physician request.
6. Reporting of home health measurements. (Such as blood pressure and glucose levels. )

Patients full name and birth date must be in the body of the message.

This e-mail will be handled by our office staff and then directed to the physician as needed. E-mail becomes a permanent part of your medical record and must only reflect one patient per message.

This service is not meant for solicitation or advertisement purposes. This e-relationship will be terminated if personal messages are received (i.e. jokes, chain letters, or stories.)

PPCP is not liable for network infractions beyond our control. Every attempt will be made to respond to your e-mail within one business day.
